An observation can provide much interesting information about work processes, design and processes, but a screening with a systemic approach will also be capable of revealing underlying causes of breaches of patient safety, near-accidents and inefficient work processes.

Human Factors behind accidents

In 2006, more than 12,000 adverse events were reported to the Danish Patient Safety Database. Most of these adverse events were due to Human Factors.

Therefore, many resources are to be found in a systematic approach to safety efforts - resources that can be utilised for improved safety as well as better quality treatment.
